Why Volunteer?

• Help individuals regain confidence and independence
• Reduce hospital readmissions through essential support
• Gain valuable experience in healthcare & community services
• Receive full training and ongoing support

What Will You Do?

• Assist patients from hospital to home or meet them at home
• Help with food shopping & home safety checks
• Provide emotional support & connect patients with local services
• Offer 1 to 2 hours a week of support for up to 28 days post-discharge

Who Can Join?

• Compassionate individuals with good communication skills
• Reliable and passionate about helping others
• Basic internet skills and access to a smartphone or email
